This will help them give you the best treatment and care, where and when you need it.\u003c\/strong\u003e\u003c\/p\u003e\n\u003cp\u003eWe’re doing this through a new health information connector called the Shared Digital Health Record. This allows your important health information - like your health conditions and medications - to be securely shared between your healthcare providers.\u003c\/p\u003e\n\u003ch3\u003eInformation to be shared\u003c\/h3\u003e\n\u003cp\u003eThe Shared Digital Health Record will collect and share health information held in places like your general practice, pharmacy, urgent care, telehealth and hospital services. It will also share information from Health NZ national datasets such as immunisations, medications and diagnostic results.\u003c\/p\u003e\n\u003cp\u003e\u003cstrong\u003eYou don’t need to do anything if you’re happy for your health information to be shared through the Shared Digital Health Record.\u003c\/strong\u003e\u003c\/p\u003e\n\u003cp\u003eYou can also choose to limit or block your information from being shared.
