Parents’ Evaluations of Developmental Status-Revised (PEDS-R) is a developmental assessment tool licensed for limited use in New Zealand from The Royal Children's Hospital, Melbourne. This resource is a fillable PDF of the PEDS-R booklet, available for download with the following requirements for use. For professionally printed paper copies of the PEDS-R booklet, orders can be made by emailing healthed@tewhatuora.govt.nz

By downloading the PEDS-R fillable PDF Booklet, you are agreeing to the following: 

  1. PEDS-R® is a tool for clinicians who have been trained in its use, to support discussion with whānau to help identify developmental concerns.
  2. PEDS-R® is only to be used in the delivery of Well Child Tamariki Ora (WCTO), B4 School Check (B4SC) or Gateway Assessment services in New Zealand.
  3. PEDS-R® can only be used by those who have successfully completed the required PEDS-R® online training available through The Royal Children's Hospital of Melbourne, Australia (*if you have not completed the PEDS-R® online training please email PEDSR@tewhatuora.govt.nz for information).
  4. The PEDS-R® fillable.pdf booklet is for completing and saving on computer only. It is not for use as a paper version and cannot be printed. For professionally printed paper version of the PEDS-R® booklet email your order to healthed@tewhatuora.govt.nz
  5. This PEDS-R® fillable pdf booklet is not to be emailed to any third party including not to families, parents, relatives nor carers. 
  6. The PEDS-R® Directions must always be used when administering PEDS-R® 
  7. Once you download the PEDS-R® fillable pdf booklet from HealthEd, it needs to be:
    • securely stored and retrieved from a password protected site
    • completed PEDS-R® fillable pdf booklets saved in electronic health records
    • only used in and with the Well Child Tamariki Ora, Before School Checks and/or Gateway assessments.
