This certificate can help whānau ensure their child is fully up to date with all immunisations and will have protection if they are exposed to a vaccine preventable disease.

Vaccinator to complete information on early childhood immunisations, according to the National Immunisation Schedule. If not confident about receipt of all doses required to be fully immunised, either give necessary dose(s) or record as ‘not fully immunised.’

immunisation records for a child whose records are linked to your My Health Account.

Getting enrolled with a general practice (GP)

It’s free to enrol with a general practice – and many services for tamariki aged under
14 are free, including immunisation.

General practices normally charge a higher fee, often called a casual rate, for patients that
aren’t enrolled with their practice.

Next steps in your child’s immunisation journey

Vaccinations are offered at different stages of your life, not only during early childhood.
Your child will next be invited to receive immunisations while they are at school.
They may also be offered vaccines at other times of their life, for example if they are
going to be living in boarding schools or halls of residence. Annual flu vaccines are
recommended for all children from the age of 6 months and are free for some groups.
COVID-19 vaccines are also recommended for some groups. For information on other

  • Call the Vaccination Helpline 0800 28 29 26 to book or ask vaccination questions
    over the phone from 8.30am to 5pm, Monday to Friday (except public holidays).
    Interpreters are available and you can choose to speak with a Māori advisor,
    a disability advisor, or use New Zealand Relay Services They can also support whānau to access immunisations if they don’t have a GP or can’t get an appointment.
